These areas included breast and colorectal cancer screening, recommended vaccinations for older adults, blood-pressure control, and medication management for cardiovascular conditions.
The plan’s efforts to support participants in areas reflected in these results included initiatives to:
- Care coordination aimed at identifying and closing preventive-care gaps, including cancer screenings.
- Individualized chronic-condition management focused on blood-pressure control, medication management and coordinated care.
“Our participants depend on us to help them receive the care and support they need to live safely and independently,” said Ryan Johnson, market president of AmeriHealth Caritas’ Pennsylvania CHC plans. “This rating reflects the work of our associates and our strong partnerships with providers, caregivers, community partners, and the Department of Human Services’ Office of Long-Term Living in delivering preventive care, managing chronic conditions, and responding to the individual needs of the people we serve.”
AmeriHealth Caritas Pennsylvania Community HealthChoices serves Pennsylvanians 21 and older who are eligible for both Medicaid and Medicare or who qualify for long-term services and supports because they require the level of care provided by a nursing facility. Its service area includes Pennsylvania counties outside the five-county greater Philadelphia region. The plan has served participants since Pennsylvania’s CHC program began in 2018.
The rating comes as Pennsylvania prepares for its next procurement of CHC contracts for managed long-term services and supports for older adults and people with physical disabilities.
